You could probably just refer to this as a Hazmat suit, since thats what it sounds like you are going for.

Not really following this sentence, are you trying to say that the project head is notified about a suit's failure after you document the failure with them?
Other than those qualms, the containment procedures do draw me in, so kudos there for giving the description a good set up.
The details of the delusion appear unique to each person so affected, though consistent themes include: meeting in post secondary school, a long emotional relationship prior to marriage, frequent arguments late in the marriage and infidelity on the part of the affected individual.
I feel that this list would be more effectively parsed if it was in a bullet point list, but thats just a stylistic choice, and is not crucial.
With respect to the first addendum, I'm not really feeling it. It not only breaks the flow of the narrative, but also comes across as a little lolFoundation for my tastes.
The second addendum does kind of set up a backstory for the photograph, and does pull me in to the narrative a bit, but between the formatting being a little clustered, and all those redactions and black boxes, the whole thing was a little hard to read. Maybe consider removing some of the redactions and reformatting?
As for the third addendum, thats a nice reveal there at the end, and really drives the piece home for me.
Overall, with some polishing, I think I would feel comfortable upvoting this.
