I would love to see this done in whatever animation style you can, mainly because doing it with real actors in any decent way would probably be expensive. Basically my idea was for something very movie-trailer like. In my head, it's long, slow shots of halls and rooms of what may be a hospital or school, all very normal, except for every now and then there's a splotch of rot on a door, or a hand with a small blood trail hanging out a doorway. With this are several jump-cuts, with people being scared, running, moments of more "normal" life, maybe someone approaching a broken railcar, seeming confused about something moving inside. It speeds up a bit then, more jump-cuts of afraid people, running, someone being pulled through a wall, someone crying with large parts of their body missing and rotten, and a few short glimpses of 106, maybe just arms or a vague shape. Then, at the end, a shadowy view of the old man's face with that frozen grin, which slides back in to the darkness…then The Foundation swims up from the darkness, followed by a fade out, or maybe one more shock jump cut.
Where i was going with the Dead Space refrence as well was that I think it would be unsettling as shit to have this all be more or less slient, with a very old, broken, faltering voice singing as the main audio. Maybe something like "Bring Back My Bonnie" or some such.
